What do I need in order to qualify?
You must have up-to-date medical records within the last 12 months in order to qualify for a Medical Marijuana Card.
Qualifying conditions include:
• Human immunodeficiency virus
• Acquired immune deficiency syndrome
• Hepatitis C
• Amyotrophic lateral Sclerosis
• Crohn’s disease
• Agitation of Alzheimer’s disease
• Cachexia or wasting syndrome
• Severe and chronic pain
• Severe nausea
• Seizures, including those characteristic of epilepsy
• Severe or persistent muscle spasms, including those characteristics of multiple sclerosis
• Post Traumatic Stress Disorder
If you do not have any medical records dated within the last 12 months, and your condition pertains to the skeletal or muscular system, you can see our pain specialist to get your records up-to-date.
See The Doctor
Once you have your medical records, it's time to see The Marijuana Doctor.
The Marijuana Doctor will review your records to ensure that you are a good fit for the Medical Marijuana Program.
If you do not qualify for any reason, you do not pay for the visit.
After you have received a certification from our Doctor, you are considers qualified to become a Medical Marijuana patient here in the state of Arizona.
The next step is to submit your application to the state of Arizona to receive your medical marijuana card in order to legally purchase medicine.
When submitting the application to the state, there is a fee. The state of Arizona charges $150 to process Medical Marijuana applications. If you are part of the Food Stamp program, the State Charge is $75.
